About the role
As a Case Manager, you will be responsible for managing and coordinating services for clients in need. This includes assessing needs, developing plans, and ensuring that clients receive appropriate support.
Responsibilities
- Assess client needs and develop service plans
- Coordinate services and resources for clients
- Monitor progress and adjust plans as necessary
- Communicate regularly with clients and their families
- Ensure compliance with relevant policies and regulations
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- Minimum 2 years of experience in case management or related field
- Strong interpersonal and communication skills
- Proficient in Microsoft Office Suite
Qualifications
- Valid driver’s license and access to reliable transportation
- Ability to work flexible hours including evenings and weekends
