About the role

The Case Manager plays a crucial role in the continuum of care provided by The Institute for Human Services (IHS), a leading human services agency in Hawaii. IHS aims to end and prevent homelessness through a range of services including emergency shelters, housing programs, health services, employment assistance, and outreach.

Responsibilities

  • Demonstrate excellent customer service consistent with the Vision, Mission, and Values of the organization.
  • Manage a caseload and ensure positive outcomes through the timely coordination of all case management services with a warm and welcoming attitude.
  • Meet contract standards and provide linkages to substance abuse or mental health treatment as needed, and transportation to other involved or needed providers to coordinate member’s care and housing solutions.
  • Provide supportive counseling and participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals; coordinate written crisis plans when indicated in coordination with the client's psychiatric provider.
  • Timely and accurately document clinical records in the required systems of record and maintain records of referrals, assessments and placement plans in compliance with IHS standards of documentation and all contract requirements.
  • Make collaborative outreach attempts, including transportation of clients to other involved or needed providers to coordinate member’s care as necessary.

Requirements

  • Knowledge of homelessness and the issue associated with it; mental health and substance abuse issues.
  • Strong written communication and interpersonal skills with an emphasis on conflict resolution.
  • Excellent time management skills with ability to manage a high volume of cases.
  • Possess personal qualities of integrity, patience and commitment to mission.
  • Flexible and able to multitask; can work within an ambiguous, fast-moving environment.

Qualifications

  • High School diploma or GED equivalent (Bachelor's preferred).
  • One to two years of direct case management or supportive services experience working with homeless or other disenfranchised individuals.
  • Valid Driver’s License and clean traffic abstract.

Skills

  • Knowledge of homelessness and related issues.
  • Experience in mental health and substance abuse services.
  • Excellent communication and interpersonal skills.
  • Time management and organizational skills.
  • Ability to work in a flexible, fast-paced environment.

Benefits

IHS offers a competitive compensation package starting at $21.15 hourly, with actual pay commensurate with experience, education within contract budgets. The position is full-time, regular, and includes general business hours with some flexibility.

Schedule

Regular Full-Time classification. General business hours, Monday through Friday, including some evenings, weekends and holidays. Hours and days may vary based on the needs of the organization.
