About the role

Responsibilities

  • Provide services at the office, in the community, and in clients' homes.
  • Drive to and from off-site locations to provide services using a personal vehicle.
  • Transport clients and/or family members in a personal vehicle (mileage reimbursement included).
  • Offer case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
  • Participate in meetings, such as Treatment Team Meetings, to ensure clients' needs are met and recommendations are followed through.
  • Document all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
  • Assist the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
  • Monitor the clients' progress toward goals outlined in their treatment plans.
  • Inform the primary therapist of progress.

Requirements

  • Education & Experience: One of the following pathways is required:
    • An associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting.
    • A bachelor’s degree in a social science field plus six (6) months of experience (preferred).
    Relevant internship experience may be substituted for a portion of the required work history.
  • Licensure & Compliance: A valid California Driver’s License and eligibility to meet agency driving criteria.

Qualifications

  • Must be able to pass a background check.
  • Must be able to pass a drug screen.
  • Must be able to pass a motor vehicle record check.

Skills

  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Proficiency in Microsoft Office Suite.
  • Knowledge of mental health and behavioral health principles.

Pay

$22.00 - $28.00 USD per hour.

Schedule

Monday-Friday, 8am to 5pm. Working hours are approximate and may include flexibility as needed to meet client needs.
