About the role

Responsibilities
- Provide services at the office, in the community, and in clients' homes.
- Drive to and from off-site locations to provide services using a personal vehicle.
- Transport clients and/or family members in a personal vehicle (mileage reimbursement included).
- Offer case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
- Participate in meetings, such as Treatment Team Meetings, to ensure clients' needs are met and recommendations are followed through.
- Document all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
- Assist the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.
- Monitor the clients' progress toward goals outlined in their treatment plans.
- Inform the primary therapist of progress.
Requirements
- Education & Experience: One of the following pathways is required:
- An associate degree in a social service field plus two (2) years of experience in a mental/behavioral health setting.
- A bachelor’s degree in a social science field plus six (6) months of experience (preferred).
- Licensure & Compliance: A valid California Driver’s License and eligibility to meet agency driving criteria.
Qualifications
- Must be able to pass a background check.
- Must be able to pass a drug screen.
- Must be able to pass a motor vehicle record check.
Skills
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.
- Knowledge of mental health and behavioral health principles.

Pay
$22.00 - $28.00 USD per hour.
Schedule
Monday-Friday, 8am to 5pm. Working hours are approximate and may include flexibility as needed to meet client needs.
