Valid California Driver’s License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.
This position has primary responsibility for case management services to clients and their families referred through Stars’ Transition Age Youth (TAY) Program (TAY).
Outpatient, community-based program that serves youth age 18-25 with serious and persistent mental health issues.
Program provides intensive case management, outreach and engagement, medication support, as well as mental health counseling, peer groups, and supportive services to assist TAY transition to adult life.
Services offered are trauma informed and culturally responsive.
