Responsibilities

• Reviewing emails and phone messages
• Checking and following up on referrals
• Reviewing and ensuring proper documentation and consents are completed and signed
• Conducting discharge planning
• Conducting utilization review
• Working well with other case managers
• Demonstrating strong communication skills
• Working with a team

Qualifications

• Current California RN license
• 3 years of experience as an RN

Pay

Compensation is commensurate with experience.
