Case Manager

Responsibilities

  • Build professional relationships with clients and maintain confidentiality
  • Review client eligibility documents and explore non-shelter alternatives
  • Create and manage client files and records
  • Conduct intake interviews and psycho-social assessments
  • Develop and update Independent Living Plans with short- and long-term goals
  • Refer clients to permanent housing and community-based services
  • Support clients in overcoming barriers and advocating for themselves
  • Track client progress and document updates through regular contact
  • Follow up with clients and referral agencies to monitor service outcomes
  • Provide data for required reports (weekly, monthly, quarterly, annual)
  • Act as a liaison with outside organizations for education, healthcare, childcare, housing, and legal matters

Qualifications

  • A High School Diploma or equivalent required
  • An Associate or Bachelor’s Degree in Human Services, Social Work, or a related field strongly preferred
  • At least 1 year of experience in case management or social services working with individuals experiencing homelessness, substance use, mental health challenges, or other complex needs
  • Strong interpersonal skills and the ability to build trust with individuals from diverse backgrounds
  • Clear and effective communication skills for conducting interviews, documenting progress, and coordinating with external partners
  • The ability to maintain accurate records and meet reporting deadlines
  • A commitment to maintaining client confidentiality and adhering to professional ethics
  • Familiarity with New York City’s housing, healthcare, legal, childcare, and educational resources
  • Problem-solving skills and a supportive approach to helping clients overcome challenges
  • Organized and detail-oriented, with the ability to manage multiple responsibilities
  • Culturally sensitive and responsive to the needs of individuals and families experiencing homelessness

Additional Information

The Case Manager role is a full-time non-exempt role with an hourly rate of $27.03 (annualized at 35 hours a week to $49,209) with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match, and tuition reimbursement. This role is located in Clifton, Staten Island and has a schedule of Sunday-Thursday 4pm-12am.
