Job Summary
The Case Manager is a Registered Professional Nurse responsible for providing care in a specialized clinical area, ensuring desired outcomes, and coordinating care.
Responsibilities
- Accountable for desired clinical and cost-effective outcomes
- Collaborative practice and resource allocation
- Discharge planning and care evaluation
- Improving quality and efficacy of care
- Implementing case management processes
- Ensuring adherence to case management standards and competencies
- Assessing and interpreting patient data
- Providing care appropriate to the patient's age and unique needs
Requirements
- Bachelor of Science in Nursing degree-preferred
- Licensed to practice as a Registered Nurse in the State of Missouri
- BLS Certification
- Three (3) years clinical nursing experience
Job Details
Type: Full Time (80 hours per 2 week pay period, with benefits)
Typical hours: Monday - Friday 8:00am - 4:30pm; Weekend days rotation
