Case Manager

Job Summary

The Case Manager is a Registered Professional Nurse responsible for providing care in a specialized clinical area, ensuring desired outcomes, and coordinating care.

Responsibilities

• Accountable for desired clinical and cost-effective outcomes
• Collaborative practice and resource allocation
• Discharge planning and care evaluation
• Improving quality and efficacy of care
• Implementing case management processes
• Ensuring adherence to case management standards and competencies
• Assessing and interpreting patient data
• Providing care appropriate to the patient's age and unique needs

Requirements

• Bachelor of Science in Nursing degree-preferred
• Licensed to practice as a Registered Nurse in the State of Missouri
• BLS Certification
• Three (3) years clinical nursing experience

Job Details

Type: Full Time (80 hours per 2 week pay period, with benefits)

Typical hours: Monday - Friday 8:00am - 4:30pm; Weekend days rotation
