About the role
This is an exciting opportunity to make a real difference in the lives of our patients as a Case Manager at Perimeter Behavioral of Missouri - Saint Robert. In this role, the Case Manager will be coordinating care and communication for adolescents with behavioral challenges and medically complex physical conditions.
Responsibilities
- Serve as the main contact from adolescents and their families, offering support and guidance.
- Assess the needs of adolescents with behavioral and medical conditions and develop individualized care plans in collaboration with the healthcare team.
- Monitor patient progress and outcomes, adjusting care plans as needed to meet evolving needs and goals.
- Advocate for the rights and needs of patients and families, assisting them in navigating the healthcare system and accessing necessary resources.
- Provide education and support to families regarding treatment options, coping strategies, and community resources for ongoing support.
- Maintain accurate and confidential case documentation, including care plans, progress notes, and communication logs, in accordance with hospital policies and regulatory requirements.
Requirements
- Bachelor’s degree in social work, psychology, nursing, or related field.
- Valid Missouri Driver's License is preferred
- At least twenty-one (21) years of age
Qualifications
Experience in a mental health setting (preferably with adolescents), and knowledge of child/adolescent growth and development, and the ability to relate to adolescents with substance abuse or mental health problems is preferred.
