Case Manager

Responsibilities

• Regular apartment visits and inspections
• Medication and medical monitoring in conjunction with care coordinators
• Linkage to community resources
• The facilitation of client empowerment and choice
• Contact with therapists, psychiatrists, peer specialists, and other natural supports

Requirements

• Bachelor’s degree in the Human Services field (Masters preferred)
• Full-time position (some evening hours required)

Benefits

• Bachelors Level: $55,000
• Masters Level: $65,000
