Core Responsibilities

• Assess clients' needs and resources
• Coordinate with licensed therapists in developing treatment plans and goals
• Maintain client records and ensure compliance
• Verify funding eligibility for Medicaid and manage status
• Arrange client appointments and follow up
• Document services accurately and promptly
• Communicate with collateral supports
• Complete accurate and thorough client reports
• Provide positive reinforcement for client successes

Requirements

• Licensure / Education / Prior Experience: Bachelor's degree in a related field, two years of related employment, and/or program graduate recommended by staff for employment in pursuit of licensure preferred.
• Background check and First Aid/CPR certification required.
• Annual TB tests, flu, and other immunizations as required.
• Employment eligibility.
• Able to work variable shifts as needed.
