About the role
As a Case Manager at Neulife Rehabilitation, you will play a vital role in providing support and guidance to individuals in need of rehabilitation services.
Responsibilities
- Conduct assessments and evaluations to determine individual needs
- Develop and implement individualized treatment plans
- Cookordinate and monitor the delivery of services to clients
- Collaborate with healthcare professionals and service providers
- Advocate for clients' needs and rights
- Maintain accurate and up-to-date documentation
Requirements
- Bachelor's degree in social work, psychology, or related field
- Previous experience in case management or a related role
- Strong interpersonal and communication skills
- Knowledge of community resources and support services
- Able to multitask and work in a fast-paced environment
