Case Manager

About the role

This position plays a critical role in supporting the rehabilitation process of clients at Neulife Rehabilitation Center.

Responsibilities

• Assess client needs and develop personalized care plans.
• Coordinate with medical staff to ensure seamless integration of care.
• Provide emotional support and counseling to clients.
• Monitor client progress and adjust care plans accordingly.
• Document all interactions and progress updates in client files.

Requirements

• Bachelor’s degree in Psychology, Social Work, or related field.
• At least 2 years of experience in a healthcare setting.
• Valid driver’s license and access to reliable transportation.
• Excellent interpersonal and communication skills.
• Ability to work flexible hours including evenings and weekends.

Qualifications

• Proficiency in Microsoft Office Suite.
• Knowledge of rehabilitation techniques and principles.
