About the role

Neighborhood Outreach Access to Health (NOAH) is a Federally Qualified Health Center (FQHC) that offers comprehensive, integrated, and affordable healthcare services to people in need. We serve over 40,000 neighbors with a variety of services, including medical, dental, behavioral health, nutrition, preventive health, eligibility assistance, and health education programs.

The Case Manager is a Behavioral Health Technician (BHT), under the direct supervision of a licensed behavioral health professional (BHP), providing clinical services that target treatment outcomes and promote the biopsychosocial health and wellness of the client. This supportive role improves treatment outcomes and helps patients meet their treatment plan goals.

Responsibilities

  • Assist in identifying, implementing, maintaining, monitoring, and/or modifying behavioral health services on a routine basis.
  • Identify, find, and implement necessary resources that support activities of daily living and/or respond to Social Determinants of Health (SDOH) beyond behavioral health services.
  • Coordinate care with the patient, their Health Care Decision Maker (HCDM), healthcare providers, family, community resources, and other support systems, including educational, social, judicial, and state agencies.
  • Ensure continuity of care between levels of care (e.g., inpatient to outpatient) and across multiple services (e.g., personal care, housing, nursing, and family counseling) and providers.
  • Assist patients in applying for social security benefits.
  • Conduct outreach and follow-up with patients after a crisis event.
  • Participate in formal and informal case staffing, case conferences, and other meetings with or without the patient/HCDM or their family.
  • Support, collaborate, and manage caseloads with assigned clinicians/providers to address identified needs.
  • Assist in maintaining, monitoring, and modifying covered services as outlined in the patient’s service plan to address clinical needs.
  • Support rescheduling, outreach, and reengagement efforts as needed.
  • Adhere to all rules and regulations regarding patient confidentiality and patient rights as outlined in contracts, NOAH policy, and state/federal regulations.
  • Maintain position productivity expectations.
  • Perform other related duties as assigned to address or support patient treatment needs.

Qualifications

Required Skills/Knowledge/Abilities:

  • Knowledge of case management principles and practices.
  • Ability to establish and maintain effective working relationships.
  • Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds.
  • Ability to understand and follow written and verbal instructions.
  • Excellent organizational skills and attention to detail.
  • Ability to exercise confidentiality, discretion, and good judgment.
  • Proficient with Microsoft Office Suite or related software.
  • Ability to read, write, and speak English; Spanish proficiency preferred.

Education and Experience:

  • High school diploma or equivalency, plus a combination of education in a behavioral health-related field and at least two years of full-time behavioral health work experience.
  • Associate’s degree and at least two years of full-time behavioral health work experience.
  • Bachelor’s or master’s degree plus one year of behavioral health experience or a bachelor’s or master’s degree with a behavioral health focus.

Other Requirements

  • New hires must pass a pre-employment background check and drug testing (effective 11/1/2022).
  • Current Level 1 Fingerprint Clearance Card or must obtain it within 7 days of hire.
  • Basic Life Support (BLS) certification.
