Case Manager
This role is based in the Northwest/Middle TN area of the state: Montgomery, Cheatham, Stewart, Robertson, Sumner, Davidson, and Dickson counties. This is a hybrid position blending remote work and in-office duties with occasional requirements to report to the facility. The role requires full-time dedication and flexibility to include weekends and evening work.
About the role
Universal Health Services, Inc., in partnership with Greystone Foster Care, provides comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. Greystone Foster Care recruits and trains foster parents to provide a safe, stable, and therapeutic environment for children and youth who have been removed from their homes. The program prepares staff and foster parents to support children and youth as they begin the healing process, linking them to hope, healing, and trust.
Responsibilities
- Complies with state regulatory requirements and the Greystone Foster Care plan
- Ensures all placement documentation is completed according to established timelines
- Completes regular file audits for each child and participates in peer file audits to ensure audit preparedness
- Assists with completing foster parent reassessments when necessary
- Schedules child/family team meetings at least quarterly and ensures corresponding documentation is completed and submitted
- Visits each assigned child per policy or as needed
- Ensures annual EPSDT and biannual dental appointments are scheduled and information is provided to DCS
- Ensures outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs
- Obtains required consents and releases from appointed guardians for medical appointments and medications
Requirements
- Bachelor's degree in Social Work or related field; Master's degree preferred
- One to three years' related experience and/or training, or equivalent combination of education and experience
- Valid state Driver's License
- Automobile insurance with verification of medical liability
- Successful background check, fingerprinting, and employment verification
