Case Manager
Pay: $16.50 - $18.00 per hour

Responsibilities
- Follow up with Admissions Coordinator to ensure consent forms are signed
- Orientate the students to MMA expectations, intake packet and campus
- Update student info in database on intake and throughout student’s stay at MMA
- Provide transportation to current students to court, home passes, etc., as needed
- Appear in court as needed, telephonic or in person
- Relay information to caseworkers, ensuring good communication between MMA and agency
- Participate in CFT meetings and act as a liaison for MMA and outside agency
- Assist Marketing/Admissions Department when needed
- Responsible for Clinical charts (correct order, needed paperwork, review monthly)
- Facilitate students staffings (service plans, signature pages, notes)
- Compile and submit monthly referring/placing agency reports
- Send information as requested to agencies
- Assist students in preparing for discharge in conjunction with agency guidelines
- Any other duties assigned by the supervisor
Requirements
- Must be at least 21 years old
- Must be able to pass a thorough background check, and obtain a Level 1 Fingerprint Clearance Card
- High School diploma or GED required, plus a minimum of two (2) years of combined education and/or relevant experience in human services, behavioral health, social services, or a related field
- Experience with youth and/or behavioral health preferred

About Mingus Mountain Youth Treatment Center
Mingus Mountain Youth Treatment Center (MMYTC) is a nonprofit residential behavioral health treatment center serving adolescents with serious emotional, behavioral, and substance use disorders. Located in Arizona, we provide structured, trauma-informed, and clinically sophisticated care for youth who require intensive therapeutic support.
MMYTC staff routinely treat vulnerable youth who have survived abuse, neglect, and exploitation in their communities, which provides a unique opportunity for staff to provide life-saving care.
