Case Manager

About the role

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in various settings, including community living arrangements, schools, and natural support residents.

Responsibilities

• Assess and plan recovery treatment and support plans for clients
• Implement and transfer skills identified in the recovery treatment and support plan
• Collaborate with other members of the treatment team to provide intensive therapeutic services
• Work in a variety of settings including community living arrangements, schools, and natural support residents

Requirements

• Bachelor’s degree in Social Work, Psychology, or another human service-related field required
• Valid PA driver’s license with a clean driving record required
• One year of experience in the behavioral health field preferred

• Clearances and background checks must be completed at Merakey’s expense
