Case Manager

About the role

Case Manager/Parent Coach will work with families who are involved with the ODHS Child Welfare system and/or reside in the Transitional Treatment Recovery Services program. As a "Safety Service Provider," you will provide transportation and services in the community and in the family’s home, including hands-on parent coaching, education, interventions, and skills training to achieve behavioral changes in families where child safety issues are present.

You will attend meetings and collaborate with ODHS Case Workers, community partners, and other agencies, providing detailed and timely oral and written reports. The role involves using evidence-based curriculum to facilitate parent education groups in a community living environment and delivering a range of in-home or community-based services such as parent education, modeling, mentoring, communication skills, self-care, home management, and nutrition.

Responsibilities

• Assess parental capacities related to child safety, well-being, and needs to help resolve child and family safety and attachment concerns.
• Identify and create goals in partnership with ODHS Child Welfare and clients.
• Provide supervised visits to children separated from one or both parents to ease transitions from resource parents or relative placements to home.
• Occasionally provide respite care in the home or community, including recreational activities and behavioral monitoring.
• Prepare curriculum and teach parenting groups as needed.
• Work a flexible schedule to meet families’ needs, including occasional weekends, evenings, and monthly weekend on-call rotation.
• Coordinate access and provision of services from multiple agencies, including establishing service linkages, advocating for service needs, and obtaining entitlements.
• Work with individuals in difficult circumstances, including angry or hostile clients, disadvantaged families, and those with mental health or developmental challenges.
• Identify families with drug and alcohol dependence, physical abuse, or sexually abused children.

Requirements

• Bachelor’s degree in social work, psychology, counseling, or a related field, or equivalent experience.
• Two years of experience working with high-risk families within the last five years, including experience with sexually abused youth, victims/perpetrators of domestic violence, and alcohol/drug addictions.
• At least two years of experience providing in-home services, such as budgeting, housekeeping, nutrition, household management, physical care of children, and parenting skills.
• Standard LCSNW criminal clearance, including national sex offender registry check.
• Valid driver’s license, clean motor vehicle records, and auto insurance.
