Case Manager

About the role

LaSante Health Center is seeking a compassionate and organized Case Manager to help patients address social, financial, and healthcare-related needs. The Case Manager will connect patients with community resources, public benefits, and support services while coordinating with the clinical care team.

Responsibilities

• Assess patients’ needs and develop individualized service plans.
• Connect patients with food, housing, transportation, benefits, homecare, legal, and other community resources.
• Assist patients with applications, referrals, and required documentation.
• Follow up with patients and community organizations to ensure services are accessed.
• Coordinate care with therapists, medical providers, and other team members.
• Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
• Track patient needs, referrals, deadlines, and progress.

Requirements

• Experience in case management, social services, healthcare, or a related field preferred.
• Familiarity with New York City community resources and public-benefit programs preferred.
• Strong communication, organization, documentation, and follow-up skills.
• Ability to work compassionately with vulnerable individuals and families.
• High school diploma or equivalent required.
• Bilingual skills are a plus.
