Case Manager

About the role

The Hampton-Newport News Community Services Board is seeking a full-time Case Manager to support the Behavioral Health Docket (BHD) program. This role involves collaborating with an experienced multidisciplinary team to coordinate services for individuals with behavioral health needs.

Responsibilities

- Assess client needs and coordinate services. - Develop and monitor individualized service plans. - Connect clients with community resources. - Provide advocacy, crisis intervention, and supportive case management. - Collaborate with treatment providers, families, and criminal justice partners. - Maintain accurate documentation and client records.

Requirements

- Bachelor's degree in human services such as Social Work, Psychology, Sociology, or Counseling. - One (1) year experience in case management services. - Valid Virginia Driver's License.

Annual Salary: $50,604
