About the role
The Empowerment Center Case Manager plays a pivotal role in supporting individuals experiencing homelessness by providing comprehensive case management services.
Duties & Responsibilities
- Treat all clients with respect, inclusivity, and dignity.
- Ensure that clients adhere to established rules and that their needs are met.
- Welcome and introduce yourself to new clients, explaining the services offered by the center.
- Engage in meaningful conversations to build professional rapport with clients.
- Absorb and prepare clients for hot and cold drinks as needed.
- Ensure clients are using permitted areas in the centers.
- Conduct comprehensive intakes to gather personal information and identify client needs; accurately enter information into the database.
- Develop and perform assessments to better understand the client’s situation and future goals.
- Offer consultations and planning sessions, suggesting actionable pathways and setting goals with clients.
- Provide ongoing support through follow-up meetings and periodic check-ins.
- Assess financial needs and housing situations, providing guidance on available resources and referrals for financial assistance and housing programs.
- Aid clients in accessing services such as job searching, resume building, and obtaining necessary identification documents (IDs, birth certificates, social security cards).
- Document all client interactions promptly and accurately in the database to ensure continuity of care.
- Maintain a clean and organized workspace at the end of each shift, restocking supplies as needed.
- Perform daily cleaning tasks, including vacuuming/sweeping floors, wiping down surfaces, and maintaining the cleanliness of bathrooms and communal areas.
- Dispose of trash and ensure all areas are locked and secure at the end of the day.
- Provide support and knowledge sharing with co-workers regarding ongoing case work.
- Collaborate with colleagues to seek feedback and suggestions on client cases.
- Participate actively in staff meetings and contribute positively to discussions.
- Support and mentor interns in their learning and professional development.
Required Experience & Skills
- Relevant experience may be considered in lieu of a degree.
- Proven experience in case management or direct service roles.
- Strong verbal and written communication skills.
- Ability to establish and maintain professional boundaries while providing compassionate support.
- Familiarity with local resources for housing, financial assistance, and employment services.
- Proficient in using databases and computer software for documentation and reporting.
- High level of patience, compassion, and empathy toward clients.
- Strong work ethic and ability to maintain professionalism in challenging situations.
- Excellent organizational skills and attention to detail.
- Ability to work collaboratively with diverse teams, staff, volunteers, and clients.
Nice-to-Haves
- Certifications in trauma-informed care, mental health first aid, and substance abuse counseling.
