Case Manager

Job Responsibilities

• Conduct initial and ongoing assessments to evaluate clients' mental health, substance use, strengths, and needs.
• Develop personalized recovery plans in collaboration with clients, outlining specific goals and interventions.
• Cook up coordination with healthcare providers, social services agencies, and community resources to ensure comprehensive support for clients.
• Advocate for clients' rights and preferences within the healthcare and social service systems.
• Provide emotional support and encouragement to clients throughout their recovery process.
• Educate clients about mental health and addiction issues, treatment options, coping strategies, and relapse prevention techniques.
• Teach life skills such as budgeting, time management, and communication to help clients achieve greater independence.
• Document all interactions, services provided, and outcomes accurately and timely.
• Work closely with multidisciplinary teams including psychiatrists, therapists, nurses, social workers, and case managers to ensure integrated care.

Qualifications

• Previous experience in case management, mental health, substance abuse treatment, or related fields is preferred.
• Experience working with individuals in recovery or in crisis situations is beneficial.
• Strong interpersonal and communication skills are essential.
• Organizational skills, problem-solving abilities, and the ability to work collaboratively in a team environment are also important.
