Responsibilities
- Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.
- Provide individualized case management for all participants.
- Manage, build and create relationships with local agencies and service providers.
- Afford youth in navigating systems including providing court advocacy and transportation to medical appointments.
- Coordinate case planning/service delivery with assigned court staff, including Resource Coordinators, and make appropriate cross-system referrals.
- Engage with other community-based service providers as appropriate to ensure a wraparound approach.
- Be available to support the team in emergencies regarding incidents with staff or participants.
- Adhere to all documentation and database requirements.
Qualifications
- Bachelor’s Degree in related field with appropriate experience.
- Bi-lingual English-Spanish required.
- 2+ years of experience in working with at-risk populations.
- Proficient computer skills with MS Office.
- Valid and clean driver's license.

Schedule
Mon-Fri, evenings and weekends as needed.
