Key Responsibilities
- Assess individual's needs and develop personalized care plans
- Career Management Services
- Coordinate services and resources across multiple agencies and providers
- Maintain accurate and detailed case records
- Monitor individual's progress and adjust care plans as necessary
- Provide guidance and support to individuals navigating complex systems
- Advocate for individuals to ensure they receive appropriate support
- Collaborate with team members and external stakeholders to optimize individual outcomes
Skills And Qualifications
- Proven experience in case management, social services, or related roles
- Excellent communication and interpersonal skills
- Strong organizational and time-management abilities
- Ability to handle sensitive and confidential information with discretion
- Knowledge of community resources, healthcare systems, and social services
- Problem-solving skills and a compassionate approach to indvidual care
Requirements
- MINIMUM REQUIREMENTS: TN Licensed Practical Nurse (LPN) preferred; or Medical Assistant certificates may be accepted as an alternative to LPN with at least one (1) year of experience.
- or, an Associate’s Degree or Five (5) years of healthcare or DDA experience may be accepted as an alternate to a LPN or Medical Assistant certificate.
- Case management experience preferred.
- DDA and Managed Care Organization (MCO) designated online training, First Aid, CPR, and Medication Certification course within first 90 days of employment, and any other required training.
- Person Centered Thinking Training.
- Emory Valley Center Orientation.
- Skills System training.
- Valid TN driver’s license and a good driving record.
