Position

Full-time

Key Responsibilities

  • Assess individual's needs and develop personalized care plans
  • Career Management Services
  • Coordinate services and resources across multiple agencies and providers
  • Maintain accurate and detailed case records
  • Monitor individual's progress and adjust care plans as necessary
  • Provide guidance and support to individuals navigating complex systems
  • Advocate for individuals to ensure they receive appropriate support
  • Collaborate with team members and external stakeholders to optimize individual outcomes

Skills And Qualifications

  • Proven experience in case management, social services, or related roles
  • Excellent communication and interpersonal skills
  • Strong organizational and time-management abilities
  • Ability to handle sensitive and confidential information with discretion
  • Knowledge of community resources, healthcare systems, and social services
  • Problem-solving skills and a compassionate approach to indvidual care

Requirements

  • MINIMUM REQUIREMENTS: TN Licensed Practical Nurse (LPN) preferred; or Medical Assistant certificates may be accepted as an alternative to LPN with at least one (1) year of experience.
  • or, an Associate’s Degree or Five (5) years of healthcare or DDA experience may be accepted as an alternate to a LPN or Medical Assistant certificate.
  • Case management experience preferred.
  • DDA and Managed Care Organization (MCO) designated online training, First Aid, CPR, and Medication Certification course within first 90 days of employment, and any other required training.
  • Person Centered Thinking Training.
  • Emory Valley Center Orientation.
  • Skills System training.
  • Valid TN driver’s license and a good driving record.
  • Annual registry, background, and motor vehicle record reviews are part of our commitment to safety.

  • Strong interpersonal and communication skills, both verbal and written.

  • Demonstrated good judgment, responsibility, and the ability to work independently.
  • Relevant experience in social services, case management, or related fields is preferred.
