Case Manager

Responsibilities

  • Conduct education assessments upon intake and develop individualized Education Success Plans tailored to each student’s needs.
  • Cookordinate and facilitate weekly or as-needed check-ins with students, while maintaining communication with school contacts to support academic accommodations and track attendance.
  • Attend staff meetings, and assist with student conflict resolution and case management as needed.
  • Collaborate with the clinical team, parents, and the Admissions Director to create treatment schedules, manage pre-treatment processes, and plan for discharge.
  • Oversee medication management coordination to ensure accurate and consistent delivery of prescribed medications.
  • Provide access to essential health services by assessing student needs and working with administrative staff to schedule medical, dental, and vision appointments.
  • Maintain timely and accurate documentation in Best Notes, conduct parent outreach for student updates, and support students in learning life skills and household responsibilities.
  • Flexibility to adjust hours or provide additional support as needed based on clinic needs

Qualifications

  • Education & Experience: Bachelor’s degree in Behavioral Sciences, Education, or a related field required; experience in education and behavioral health strongly preferred.
  • Working knowledge of DSM-V disorders, ability to develop and adjust treatment plans, and experience supporting students and families in crisis with professionalism and empathy.
  • Strong verbal and written communication skills, ability to set and maintain healthy boundaries, and a calm, supportive approach to conflict resolution.
  • Prioritize tasks, meet deadlines, and adapt in a fast-paced, evolving clinical environment.
  • CPR and First Aid certified and able to pass a state background check.
