Case Manager

About the role

As a Case Manager, you will be responsible for managing and coordinating the care of clients in a healthcare setting. You will work closely with medical professionals to ensure that clients receive the best possible care.

Responsibilities

• Assess client needs and develop care plans
• Coordinate with healthcare providers to ensure continuity of care
• Monitor client progress and adjust care plans as necessary
• Communicate with clients and their families about their care
• Document all interactions and care provided

Requirements

• Bachelor’s degree in a related field
• Experience in healthcare or social services
• Strong organizational and communication skills
• Ability to work independently and as part of a team

Qualifications

• Valid driver’s license
• Background check required
