At Community Counseling of Bristol County (CCBC), we are building an amazing team of professionals who are changing the way mental health is treated in Southeastern Massachusetts. We provide services in schools, homes, the community, and our Taunton offices, empowering clients with evidence-based behavioral health treatments. As a non-profit community mental health center, we serve 50 communities in Massachusetts, offering inpatient, outpatient, and urgent care services to adults and children overcoming depression, anxiety, mood disorders, substance abuse, and other conditions.

About the Role

Be part of the innovative PACT-Y program, the first of its kind in Massachusetts, where CCBC and the Department of Mental Health (DMH) collaborate to meet the needs of youth and families. As a Case Manager, you will work within a multi-disciplinary team to improve the social, emotional, and recreational functioning of youth transitioning to adulthood. The role involves providing community-based opportunities for youth to practice skills, assist with job attainment, explore after-school activities, and support goals identified in the team treatment plan.

Responsibilities

  • Provide case management services to increase skills related to independent living, community integration, employment, recreation, and housing.
  • Assist families in obtaining resources to improve family functioning within their community.
  • Support and teach skills related to the treatment plan.
  • Model skills and techniques in the community to increase youth’s feelings of competence and self-esteem.
  • Ensure the youth has a voice in treatment planning and identification of goals.
  • Complete required documentation per program guidelines.
  • Coordinate care with the family, team, providers, and DMH.
  • Provide on-call and weekend support on a rotating basis.
  • Facilitate change based on youth and family needs, strengths, and preferences.

Requirements

  • Bachelor’s degree in psychology, social work, or a related field.
  • Some prior experience working with children, youth, and families with mental health challenges.
  • A valid driver’s license and reliable transportation.
  • Willingness to learn and seek guidance as needed.
  • Good organizational skills, effective verbal, written, and computer skills.

Schedule

11:00 AM – 7:00 PM, Monday through Friday, with weekend rotation and some flexibility.

Pay

Starting salary of $50,000 with a $2,000 sign-on bonus.
