About the role
This position involves providing case management services to youth and families to develop a natural support system. Services include client assessment, monitoring progress, crisis intervention, and community referrals.
Responsibilities
- Responsible for case management of clients to achieve goals, objectives, and work plans according to established standards and policies.
- Participates in multi-disciplinary teams.
- Provides rehabilitation and support services to a caseload of youth and their families, aligning with individual treatment goals.
- Performs timely administrative tasks such as documentation of services, report preparation, and other required statistical information.
- Attends staff and individual case review meetings with the program manager.
- Networks with community agencies to link clients to necessary resources.
- Adheres to confidentiality and HIPAA regulations.
- May represent the program and present to community groups and professionals.
- May assist in program development activities as directed.
Requirements
- Education and/or Experience: Bachelor’s degree in social work, health services administration, nursing, education, health education, or related field with relevant experience in case management and working with youth.
- Knowledge, Skills & Abilities: Knowledge of Internet software, spreadsheet software, word processing software, and specialized database applications. Bilingual (English/Spanish) required, counseling and communication skills with young adults and families preferred.

- Current valid California Driver's License with eligibility for coverage under the agency insurance plan.

Pay
The starting salary range for this position is between $21.00 - $23.00 per hour.
