This position works with at-risk clients, their families, medical care teams, and Family Services program staff to identify medical, behavioral, and social needs. It provides case management interventions to achieve optimal patient outcomes.

About the Role

  • Works as part of an interdisciplinary care team to manage the needs of at-risk clients, improving access to care and outcomes.
  • Assists clients experiencing medical, behavioral, and social conditions impacting their health to achieve positive outcomes.
  • Helps clients follow through with care plans in cooperation with the interdisciplinary care team.
  • Provides patient-centered education on wellness and prevention topics.

Responsibilities

  • Coordinates medical, behavioral, and social healthcare needs, linking patients to resources (e.g., transportation, financial assistance) to achieve positive care outcomes.
  • Assists clients by answering questions and teaching them how to access community services/resources to promote self-sufficiency.
  • Engages in home and/or office visits for eligible clients.
  • Provides motivation and support.
  • Develops and maintains relationships with community agencies relevant to program functions or referral needs.
  • Manages caseload by providing timely interventions, quality services, and achieving positive outcomes while maintaining productivity levels.
  • Maintains documentation of services, interventions, referrals, follow-up care, and progress toward outcomes in accordance with program requirements.
  • Requires advanced organizational skills to manage projects or the work of others.
  • Works independently and as part of a team, setting priorities and re-prioritizing as circumstances change.
  • Devises effective solutions based on general goals and objectives.
  • Draws conclusions using inference and logic, which may differ from others’ interpretations.

Requirements

  • Equivalent of formal training in behavioral sciences and case management interventions, typically a Bachelor’s degree in social work or a related field.
  • Valid Washington State driver’s license with the ability to travel for home visits and other work-related activities.
  • Basic computer skills, including proficiency in Microsoft Word, Excel, PowerPoint, and Outlook.
  • Ability to enter and manage data in a centralized electronic health record system.
  • Bilingual skills in Spanish and English.

Physical Demands & Work Environment

  • Frequent standing, walking, sitting, use of hands/fingers, and talking.
  • Ability to lift up to 25 pounds.
  • Close vision (clear vision at 20 inches or less), distance vision (clear vision at 20 feet or more), and ability to adjust focus.
  • Frequent travel to various clinic sites; must meet company insurance liability standards to drive company vehicles.
  • Blood/fluid exposure risk: Category I tasks involve routine potential for mucous membrane or skin contact with blood, fluids, or tissue. Use of personal protective equipment (PPE) is required when appropriate.

Pay

Salary range: $25.63–$32.03 per hour, with potential for higher compensation for highly experienced candidates.
