Case Manager
About the role
As a key member of our team, you will play a vital role in providing support and guidance to clients. Your responsibilities will include conducting assessments, developing intervention plans, and facilitating sessions.
Responsibilities
- Conduct comprehensive client assessments
- Develop and implement individualized intervention plans
- Facilitate therapeutic sessions with clients
- Collaborate with other professionals to ensure holistic care
- Document progress and maintain client records
Requirements
- Bachelor’s degree in Psychology, Social Work, or related field
- At least 2 years of relevant experience in a mental health setting
- Licenses/certifications in counseling or social work (if applicable)
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Experience working with diverse populations
- Knowledge of evidence-based practices
- Proficiency in computer software, including case management systems
