Case Manager

About the role

As a Case Manager at Brandywine Counseling & Community Services, Inc., you will participate in assessment, evaluation, planning, linkages, monitoring, advocacy and treatment of assigned clients. You will provide services to clients dealing with substance abuse and/or mental health issues, and assist in the linkage of services needed by the client.

Responsibilities

• Complete a Well Being Care Management assessment on all clients, creating an Individualized Care Plan
• Maintain 75% of all client contacts through face-to-face interactions
• Aid in the linkage of services needed by the client
• Monitor and update the EMR for all services provided
• Provide transportation for clients as needed and assist with referrals to transportation agencies
• Ensure the provision of quality, sufficient, and appropriate services while maintaining high professional standards

Requirements

• Bachelor’s degree in behavioral health or a related field
• One year experience in the Behavioral Health field
• One year Case Management experience
• A valid driver's license with 2 points or less

Schedule

Monday - Friday, 7:00 a.m. - 3:00 p.m.

Pay

$21/hr. commensurate with experience
