About the role
Benchmark Human Services is seeking Case Managers to support individuals with intellectual and developmental disabilities, their families, and guardians. The ideal candidate will facilitate person-centered planning processes, complete assessments, and ensure services align with individual needs.
Responsibilities
- Complete core competency training required by the State
- Perform evaluations and assessments to meet the needs of individuals served
- Afford individuals the opportunity to understand and engage in the person-centered planning process
- Facilitate and schedule participation in person-centered planning and assessments as needed
- Complete Needs Assessment Annual Review Protocol annually with the individual and their team
- Support individuals in discovering and exploring their preferences, strengths, goals, and needs
- Facilitate the completion of the DS Budget and Service Planning Context Document
- Assist individuals in choosing team members who can contribute to the planning process
- Work with individuals to develop agendas and follow-up notes for person-centered planning meetings
- Support individuals in leading and participating in person-centered planning meetings
- Identify and discuss any necessary modifications to rights and freedoms
- Develop and update person-centered service plans and related documents
- Maintain oversight of person-centered service plan implementation, satisfaction, and health/well-being
- Respond to and address emergency situations and incidents appropriately and promptly
- Ensure services provided are consistent with the Individual Service Agreement (ISA) and the participant’s needs and desired outcomes
Qualifications
- Valid Driver's License and Auto Insurance
- Reside in the State of Vermont
- At least 1 year of relevant experience in a similar position
- Knowledge of Vermont's public service system for developmental disabilities services and brain injury services
- Ability to work collaboratively and meet individual performance goals
- Effective verbal and non-verbal communication skills
- Proficiency in using technology tools
