About the role

The Case Manager (CM) is responsible for assisting the Clinical team in coordinating, implementing and executing various patient related affairs to assist the patients while they are in treatment. The CM is responsible for recording and monitoring various communications, activities and interactions of the patients while they are housed in the facility.

Responsibilities

  • Aid in the supervision of patients' in their daily activities.
  • Conduct initial assessments and daily progress notes.
  • Maintain daily communication and treatment team meetings.
  • Document and facilitate assignment notes, activity notes, and group notes.
  • Collaborate with therapists for discharge planning and instructions.
  • Handle patient phone calls and external provider communications.
  • Assess clients' legal, vocational/educational, and basic life needs.
  • Facilitate Coordination of Benefits documents.
  • Complete Family Pre-assessments.
  • Support possible discharges, transfers, and AMA (Leaving Against Medical Advice).
  • Ensure patients adhere to clinical schedules.
  • Organize and facilitate patient tours of the treatment facilities.
  • Redirect and report patients who violate rules.
  • Seek corrective criticism and evaluate suggestions objectively.
  • Maintain acceptable overall attendance.
  • Promote a favorable/positive work atmosphere.
  • Attend in-services and educational training as necessary.
  • Seek out learning experiences and incorporate new knowledge in practice.
  • Adapt to expected and unexpected changes in the work environment.
  • Report personal symptoms of suspected illness or contagious diseases to the Clinical Director.
  • Communicate effectively both orally and in writing.
  • Solve problems independently and follow through on tasks.
  • Organize and function independently in an office environment.
  • Perform other tasks and duties as assigned.
  • Complete all competency assessments as required by the facility.

Qualifications & Education

  • At least 2 years of working with people with behavioral health needs (trauma, mental illness and/or substance abuse).
  • Preferred experience in providing Case Management services in the mental health or Substance Abuse community.
  • Experience working on a multidisciplinary team.
  • Tenacity and ability to think out of the box and work creatively to engage participants.
  • Knowledge of the criminal justice system and community resources.
  • Computer literate.
  • Proficient in Microsoft Office applications.
  • Excellent verbal and written communication skills.

  • Knowledge of Joint Commission standards within the Department.
  • Proficiency in Verbal and written Communication Skills.

  • Knowledge of Drug Free Workplace Policies.
  • Knowledge of Workplace Violence.
  • Knowledge of current technology.
  • Familiarity with community resources.
