About the Role

Join our team as a Case Manager at TruHealing to work alongside the clinical team and ensure life-skills improvement and long-term sobriety for patients with Substance Use Disorder (SUD) and Mental Health Diagnoses.

Responsibilities

  • Conduct a case management interview with patients upon admission to determine needs for FMLA, Short Term Disability Insurance, and identify any pending legal issues.
  • Obtain Release of Information (ROI) for referent, family, Employee Assistance Program (EAP), and/or employer.
  • Ensure patient has notified family of arrival.
  • Produce all necessary documentation for the patient's employer, schools, legal entities, etc., including but not limited to FMLA, STD, LTD, and Treatment Letters.
  • Meet with each patient at least once weekly for case management appointments.
  • Develop a strong, ongoing relationship with patients and their families, maintaining healthy communication.
  • Develop effective aftercare plans for all patients and ensure adherence to these plans.
  • Work with the outreach team or company representative to determine an appropriate aftercare plan.
  • Update and modify patient discharge dates as determined by the Utilization Review, Clinical, and Medical teams.
  • Accurately share information in team meetings.
  • Ensure medical records are sent to the facility where a patient is discharging.
  • Facilitate assigned SUD and Mental Health groups.
  • Maintain a basic understanding of health insurance benefits, terms, and other payment methods used by the organization.
  • Remind patients of program rules and regulations as necessary and report infractions appropriately.
  • Interact appropriately with patients, showing respect while maintaining boundaries.
  • Protect the privacy of all patient information in accordance with the organization's privacy policies, federal and state law, and professional healthcare standards.

Requirements

  • College degree from an accredited university, or equivalent work experience.
  • 2+ years of experience working in the behavioral health field.
  • Bachelor-level SUD certification (LADC equivalent) or CRSW/CADC.
  • Valid driver's license required.
