Case Manager

Job Summary

Affirms patients' health status, assesses the efficacy of the care plan, and monitors the client's progress alongside patient care providers. Participates in developing and reviewing patient treatment plans with healthcare professionals. Keeps track of relevant statistics for program evaluations and client services. Performs other job-related tasks as directed.

Requirements

• Proven experience in case management, such as being a Nurse, Medical, Mental Health Care Manager, or a similar role
• Thorough understanding of case management principles, healthcare management, and reimbursement processes
• Extensive knowledge of psychological aspects of care
• Exceptional communication and interpersonal skills
• Strong organizational and time management abilities
• Familiarity with emerging professional and technical knowledge
• Effective problem-solving and multitasking capabilities
• Caring attitude with strong teamwork skills
• Valid current license, certification, or registration in a related healthcare field
• Bachelor’s degree in a related healthcare field
