Case Manager

About the role

AMG Physical Rehabilitation Hospital – Covington is seeking a Full Time Case Manager to facilitate patient adjustment post-discharge. The Case Manager oversees admission and discharge planning, case communications, interdisciplinary team conferences, insurance relations, and ensures patient rights are fulfilled.

Responsibilities

• Make special services readily available to patients, families, and significant others to facilitate their adjustment to continuity of care post-discharge.
• Oversight of effective coordination of admission and discharge planning.
• Interdisciplinary team conferences to ensure comprehensive care planning.
• Management of insurance relations and assurance through managed care systems.
• Verbal and written reporting to maintain clear communication.
• Affirmation that regulations regarding patient rights are met.

Requirements

• Licensed Registered Nurse (Preferred) or Licensed Practical Nurse.
• Minimum of six months to one year of discharge planning experience preferred.
• If LPN, supervision by RN/SW required during a 6-month training period.
• Excellent time management skills; organized and able to prioritize.
• Proficient computer skills.
• Superb written and spoken communication skills.
• Excellent interpersonal skills with colleagues and others.
• Comfortable in a fast-paced environment with multiple cases.
• Able to organize and manage large amounts of files, schedules, dates, and information.
• Self-directed and able to work without supervision.
