About the Role

The Case Manager Aftercare and FRC provides case management to After Care and Family Resource Center families and provides Foster Care case management activities as assigned and as part of the oncall rotation. After Care and FRC case management activities entail developing case plans; providing resources and referrals; contributing to building strong families; providing tangible support to families; managing the Casa de Esperanza pantries; and conducting outreach to network with other organizations.
