About the role

At Adult Outpatient Valley West , you'll help clients build the skills, confidence, and resources they need to thrive. As a Case Manager, you'll provide compassionate, goal-focused support while partnering with clients to promote recovery, independence, and long-term success.

Responsibilities

  • Provides psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals.
  • Supports clients in following individualized care plans and identifying case management goals.
  • Completes documentation on services provided within Valley’s standards.
  • Mets productivity expectations as assigned by supervisor.
  • Distributes medications as necessary.
  • Transports clients to appointments as necessary.

Requirements

  • High School diploma or equivalent
  • Case Manager certification
  • CPR certification
  • Valley de-escalation certification
  • Driving positions require a minimum age of 21 and a current driver’s license

Pay

Dependent on certification status
- Uncertified Case Manager range starts at $17.50 - Uncertified Case Manager will obtain certification within the first 120 days and pay will be increased
- Certified Case Manager range starts at $18.75 - Certified Case Manager pay is calculated based on years of certified experience

Tuesday – Friday | 9:30am – 8:00pm
