Critical Duties
- Crisis Triage & Intake: Conduct preliminary screenings and triage for emergency interventions; provide immediate information and referral services to clients and families.
- Comprehensive Assessment: Perform in-home and in-office assessments of social and emotional needs using standardized instruments (e.g., PSA, EISEP-PATH).
- Case Planning & Implementation: Develop written case plans including individual/family counseling, benefit screening, and arrangement of services (Home Health Aides, Legal Aid, Housing, etc.).
- Financial & Legal Protection: Evaluate clients' capacity to manage their affairs; assist in securing financial management or guardianship to prevent elder abuse and financial exploitation.
- Field Monitoring: Conduct ongoing in-home visits to monitor risk, evaluate functioning, and revise case plans in real-time.
- Emergency Response & On-Call: Participate in a rotating on-call schedule (evenings/weekends/holidays). Respond to "cold weather days" and crisis situations requiring emergency medical or court-ordered actions.
- Documentation: Complete required case records, reports, and statistics within mandated timeframes utilizing specialized software.
Qualifications
- Education: Graduate of an accredited college or university with a BSW or BA/BS (Psychology or Gerontology preferred).
- Technical Skills: Computer literacy and the ability to maintain meticulous records.
- Communication: Proficiency in English required; knowledge of Yiddish, Russian, Spanish, or Patois is helpful.
- Physical Requirements: Ability to travel to clients’ homes using mass transit and navigate physical barriers (stairs, etc.) in residences.
