About the role
The Case Manager provides comprehensive case management services to individuals and their families, coordinates internal and external services, facilitates transitions, and maintains necessary resources.
Responsibilities
- Initial and ongoing collateral work with licensing agencies, funding sources, referral sources, guardians, courts, and other programs
- Coordination of internal services including quality assurance, pre-placement interviews, and intake support
- Cohort coordination and facilitation of individual transitions to new homes within the program or to other agencies
- Maintaining and/or accessing needed resources
- Collaborative participation with all members of the management team

- Involving the individual, to the greatest extent possible, and caregiver in the decision-making process

- Ensuring professional representation when representing the agency externally
- Welcoming and treating family members, guardians, and visitors with dignity and respect
- Working collaboratively with funding agencies, licensing agencies, and guardians in the best interest of the individual and alerting administrators of potential conflicts
- Maintaining routine communication with families/guardians, day habilitation program, residential supports, nursing department, providers, and other appropriate agencies
- Promoting interventions that foster independence and improve quality of life
- Maintaining professional boundaries and confidentiality with individuals, families, guardians, and other external stakeholders
- Establishing and maintaining positive collaborative relationships with families/guardians, day habilitation program, residential supports, nursing department, providers, and other appropriate agencies
Case Liaison
- Serving as the primary liaison to families/guardians, agency supporters, and provider organizations
- Maintaining routine communication with day habilitation program, residential supports, nursing department, providers, and other appropriate agencies
- Distributing satisfaction surveys to parents/guardians as per Crystal Springs procedure
General Responsibilities
- Attending team meetings, ISP meetings, in-service trainings, conferences, and seminars as required by Director of Adult Services
- Aiding in the admissions process as assigned
- Cooperating and participating in internal investigations
- Performing other reasonably related duties as assigned
Qualifications
- Bachelor's degree in a related field of study
- Minimum of two years' case management experience in the Human Service field
- Preferred knowledge of Department of Developmental Disabilities and Day Habilitation regulations and CARF standards
