Case Manager - Adult Mental Health Services (Mid-Intensity) ($3,000 Sign on Bonus)

About the role

The Case Manager for Adult Mental Health Services provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families.

Responsibilities

• Supportive counseling
• Individual and family support
• Crisis intervention
• Assessment
• Development of Individualized Service Plan (ISP)
• Psycho-education
• Referrals to clients and their families

Requirements

• Bachelor's Degree in psychology, counseling, social work or special education
• Registration as a QMHP is required
• Maintain QMHP status and all required trainings/CEUs
• At least one year of experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities

Qualifications

• Additional compensation for speaking multiple languages may be considered

Skills

• Strong communication skills
• Ability to work collaboratively with diverse teams
• Knowledge of mental health and substance abuse disorders
