Scheduled Hours: 32 Hours, 1st Shift

Case Manager Responsibilities
- Obtains documents and all relevant intake material.
- Counsels clients in individual intake evaluation sessions.
- Shares responsibility for education and other group programming activities.
- Participates in client evaluations and case conferences.
- Provides referrals for affected family members to self-help programs when applicable.
- Affirms patients' continuing care needs, contacts appropriate aftercare resources, and arranges for transfer or admission to appropriate rehabilitation facilities and/or liaisons with self-help organizations.
- Coordinates client's transportation to next level of care.
Qualifications
- An Associate's or Bachelor's Degree in Human Services or related field preferred; a High School Diploma or Equivalent required.
- A minimum of one year related experience and/or training in the area of addiction treatment.
- A valid Driver’s License in good standing preferred.
Pay Range
$21.77 – $26.54
