Case Manager_ 5116

Responsibilities

• Independently manage a caseload
• Conduct comprehensive intake assessments
• Conduct regular field visits to engage and build rapport with chronically homeless individuals
• Work with clients to develop goals and service plans
• Provide referrals and direct assistance to support clients applying and/or attending appointments for mental health services, medical care, substance treatment, vital documents, benefits, legal/immigration services, and permanent housing
• Document all interactions with or on behalf of clients, monitor and document client progress toward service plan goals
• Maintain familiarity with social services and resources available both on-site and in the community
• Cook up to 10 pounds.
• Attend regular case conferences with clients and other service providers, as required
• Attend regular supervision sessions with direct supervisor
• Drive agency vehicle to and from client locations and appointments
• Perform additional duties as assigned

Qualifications

• Bachelor’s degree in social work or a related field of study
• Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points
• One-year of case management or related work experience preferred
• Ability to engage and build rapport with populations with high support needs
• Strong oral and written communication skills
• Detail-oriented and resourceful with strong time management and organizational skills
• Proficiency in Microsoft Office suite and other standard business technology
• Bilingual/multilingual preferred, but not required

Physical Requirements

• Ability to use a computer for prolonged periods
• Ability to occasionally lift and/or move up to 10 pounds
• Ability to stand, walk, or sit for long periods of time
• Ability to bend and retrieve objects and/or documents
• Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
• Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
