Case Manager 2 - Adult Services

Responsibilities

• Provides case management to individuals/families involved with County departments, local human service agencies, the court system, or self-referrals.
• Offers support and referrals to individuals/families who are experiencing a life transition and need assistance navigating the human service system.
• Assesses challenges and barriers to receiving services.
• Builds and maintains relationships with existing service providers to help individuals/families access food, receive financial assistance, resolve homelessness or near-homelessness, and prevent utility termination.
• Advocates for individuals who are involved with the court system, have a mental illness, substance use disorder, and/or intellectual and development issues.
• Case management activities include client needs assessment, service coordination and client advocacy, goal setting, service follow-up, reassessment of needs, case notes, data entry, filing, and client confidentiality.

Other Duties

• Attends meetings, trainings, and seminars as required.
• Performs other job-related duties as instructed by the Director.
