Case Manager 124 ,

Responsibilities

• Working with and supporting family members and caregivers with an adult relative living with mental illness
• Providing case management services, including outreaches, linkages, advocacy, referrals, and general support
• Aiding in facilitating multi-family psycho-education and support groups, including researching ongoing psycho-education topics, presentations
• Ongoing and communication with open caseload
• Ensuring proper documentation in a timely and consistent manner

Requirements

• Bachelor’s degree in Social Work, Psychology, or related field with a minimum of 2 years as a Case Manager and 2 years working in the mental health field
• Some evening hours required
• Flexible schedule as some duties will take place in the evening hours
• Driver’s license with acceptable driving history a must
