Key Responsibilities

• Provide case management services to individuals residing in the Ozanam Inn Shelter and Permanent Supported Housing
• Screen, assess, and service plan for clients
• Manage relationships with various community providers
• Teach life skills to clients
• Transport clients to housing opportunities and assist with applications
• Build connections with other agencies
• Utilize HMIS (WellSky) case management software and MyEvolv
• Work on-call and evenings

Requirements

• Bachelor’s degree (B.A) from an accredited college or university, preferably in Social Work or a similar Human Service field
• At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients
• Experience with conflict resolution and mediation
• Valid driver’s license and clean driving record
