About the Role
The case manager will provide support and service linkages to residents within the program, coordinate services, and ensure quality care.
Responsibilities
- Oversee the operation of the Supported Housing program and assigned caseload.
- Drive self and/or agency vehicle throughout boroughs to visit apartments routinely, assessing, observing, and evaluating residents' physical plans, functioning abilities, and needs.
- Act as a liaison with community health, mental health, educational, vocational, and recreational programs.
- Supervise and collect urine samples, ensuring accurate testing procedures.
- Provide linkage services with detoxification and rehabilitation treatment centers.
- Assist consumers in the housing selection process, including securing furnishings, working with real estate agents, and setting up utility services (telephone, electric, etc.).
- Facilitate sublease and rental payment agreements.
- Assist with housing subsidies, food stamps, and other benefits and entitlements.
- Link consumers to community support services based on identified needs.
- Familiarize consumers with their rights as tenants and assist in tenant/landlord communication.
- Develop individual support plans addressing self-care (e.g., sobriety maintenance), money management, medication management, daily living skills, and symptom management.
- Maintain all required written documentation, including consumer and program records.
- Identify maintenance and safety needs of assigned apartments.
- Report and document all incidents to the immediate supervisor and/or Director of MHS within 24 hours.
- Serve on-call 24 hours a day, seven days a week to respond to emergencies.
- Provide weekly supervision of assigned staff, delegating tasks and offering professional direction and technical assistance.
- Participate in and attend meetings/trainings at various sites, requiring flexibility in travel routes.
- Able to lift/carry packages, boxes of supplies, and clients if necessary.
- Manage money, including counting, adding, and subtracting.
- Communicate information verbally and in writing to staff and supervisors for instructions, feedback, and work adjustments.
- Complete required training (AMAP, CPR, and First Aid) and perform techniques taught.
- Participate in activities of other staff members during absences or staff shortages.
- Represent the agency at meetings and trainings as needed.
- Work a flexible schedule, as work hours are subject to change.
- Maintain confidentiality at all times.
Qualifications
- Bachelor’s degree in Social Services preferred.
- Experience working with mental health consumers with or without co-occurring addictive disorders.
- New York State driver’s license and access to a vehicle required.
