Case Management Social Work Supervisor - York Case Management - Day

Responsibilities

• Assists in selecting staff, training/orienting personnel, evaluating performance, preparing work schedules and making recommendations for personnel actions.
• Aids in developing budgets.
• Identifies needs in program planning and facilitates program development.
• Supervises patient care, family communication, and record keeping.
• Collaborates with other hospital programs and community agencies with which patients and families are involved.
• Provides direct service to patients including interviewing patients and families, preparing social histories, identifying problems, and developing treatment.
• Prepares and maintains written documentation as required by the profession and the department.
• Provides therapeutic intervention with patient and family including individual, marital, parental, family and group therapies.
• Oversees the implementation and daily operations of the lean daily management system.
• Affirms staff engagement through leadership rounding and employee engagement survey results.

Qualifications

• Minimum Education: Bachelors Degree Required and Masters Degree Preferred.
• Work Experience: 2 years Relevant experience. Required Supervisory experience. Preferred.
• Licenses: Basic Life Support Upon Hire Required Licensed Social Worker Upon Hire Preferred.
