Responsibilities
- Conduct in-person patient interviews and comprehensive nursing assessments
- Develop, implement, and continually evaluate individualized patient plans of care
- Oversee and coordinate care provided by HHAs, LPNs, RNs, and other caregivers
- Communicate patient conditions and collaborate with physicians and other healthcare providers as patient needs change
- Educate patients and families on home health care procedures, including wound care, IV administration, and medication management
- Help reduce hospital readmissions through preventative care and continuity of care
- Monitor patient progress and make appropriate updates to the plan of care
Requirements
- Current, unrestricted RN license in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to and from patient care sites
- At least 1 year of professional RN experience in home health or a similar setting
- Previous case management or utilization review experience preferred
- OASIS experience preferred
- Practical tracheostomy and/or ventilator experience preferred, but not required
