Case Management Nurse

About the role

Full-time Case Management Nurse within the Bureau of Health Services, responsible for overseeing medical qualification assessments, annual medicals, and health clinics for FDNY personnel.

Responsibilities

• Conduct comprehensive nursing assessments
• Develop individualized treatment plans
• Determine treatment eligibility
• Explain treatments to patients
• Communicate with patients, physicians, and insurance providers to ensure successful health outcomes
• Document medical information accurately in electronic medical records
• Review test results
• Coordinate external treatment providers
• Perform telehealth screenings
• Administer injections and venipuncture as needed, following best practices and departmental policies

Qualifications

• Valid New York State RN license
• Either a baccalaureate degree and three years of clinical experience or an associate degree with five years of experience
• Additional experience may be required for higher assignment levels
