Case Management Nurse

Role: Case Management Registered Nurse Responsibilities Coordinate inpatient and observation care from admission to discharge.Assess patients' medical and psychosocial needs to ensure comprehensive care.Identify at-risk patients during care transitions and utilize tools like InterQual to monitor progress.Collaborate closely with physicians, nursing staff, and Social Services to develop safe discharge plans.Arrange post-acute services to support ongoing patient recovery.Serve as a liaison for patients and families to facilitate effective communication and continuity of care. Qualifications & Skills Registered Nurse licensure.Strong assessment and care coordination skills.Ability to collaborate effectively with multidisciplinary teams.Knowledge of discharge planning, post-acute services, and patient advocacy.
